Summary	0016699: Plugins: Extension dialog issues (missing icons / double-entries)
Description	1. Many plugins do not have ICONs, there should be default icon for plugins that do not have icon included in MMIP. 2. Codec Pack Manual upgrade installation can result in double listing (Pic Attached) 3. Codec Pack Missing its own Icon

michal 2020-06-09 14:10 developer ~0058475	2) is related to #16416, from version 3.0.3 we have different id (CodecPack -> MM5CodecPack), so you need to uninstall old codec pack first.
